Documentation Required for Credentialing

To join the Roundtrip Community, your company needs to submit a few documents for review.

How to Submit

Upload these during your application, or email them to the Transportation Team at transportation@roundtriphealth.com.

Certificate of Insurance (COI)

A COI from your insurance carrier confirms your company has active coverage — including the type, policy number, and limits. Requirements and minimum limits may vary depending on your vehicle types. Contact your insurance carrier to request one; many can send it electronically.

Your carrier may ask to add Roundtrip as an additional insured on your policy. Roundtrip doesn't require this, but if your carrier does, use: Company Name: Roundtrip Company Address: 1516 N. 5th Street, Unit 320, Philadelphia, PA 19122

Proof of Operating Authority

Required for every vehicle level your company operates, and for every state you operate in.

Business License

An active National Provider Identifier (NPI) and an active business license for each state you operate in.

Vehicle Roster

A list of your company's vehicles, used to confirm the vehicle types on your application. If your application and your roster don't match, your roster takes priority.

W9 Form

Required for billing and tax purposes, and also needed if you later enroll in direct deposit. Download a W9 form.
