What Documentation Does Roundtrip Require for Credentialing?
Learn what documentation your company needs to provide to join the Roundtrip Community.
To join the Roundtrip Community, your company will need to submit several essential documents. These documents will be reviewed as part of our credentialing process to ensure your company meets our guidelines for operation within the Roundtrip Community.
How to Submit Your Required Documents
As part of the application process, you will be prompted to upload the required documents for review. Alternatively, you may email them directly to our Transportation Team at transport@roundtriphealth.com.
Below is an overview of the types of documents that will be requested as part of your application:
Certificate of Insurance (COI)
A Certificate of Insurance, often referred to as simply “COI”, is a document provided by your company’s insurance carrier which verifies that your company has active insurance coverage. A COI usually includes key details such as the type of coverage, the policy number, and the policy limits.
- Depending on the type of vehicles your company offers, there may be additional insurance requirements and minimum policy limits that must be met.
To request a COI, we recommend contacting your company’s insurance carrier directly. Typically insurance carriers can generate an electronic copy of the COI for immediate assistance while other carriers may opt to mail a copy instead.

Proof of Operating Authority
Roundtrip requires your company to maintain and provide proof of operating authority for each vehicle level your company provides service for. If your company operates in multiple states, proof of operating authority must be provided for each state.
Here are some general guidelines based on vehicle type:

Taxi | Typically a specific license issued by your state’s Department of Motor Vehicles (DMV) or proof of seal |
Medical Sedan | Typically a specific license issued by your state’s Department of Motor Vehicles (DMV) or proof of seal
Drivers must have active first aid and CPR certifications |
Secure Psych/Constable | Drivers must have active certifications by their respective state’s Constable Education Training Board or other governing entity |
Wheelchair Van | Typically a state/local issued ID number |
Stretcher Van | Varies widely by state |
Ambulance | Proof of active ambulance licensure, typically issued by the respective state’s Department of Health (DOH) |
Business License
Roundtrip requires your company to maintain and provide proof of legitimacy of your business operations. This includes having an active National Provider Identifier (NPI) as well as an active license for the state(s) your company will be operating in.
Vehicle Roster
While Roundtrip does not have any specific requirements regarding fleet size, we do require a roster of your company’s vehicles. This is collected to cross reference your company’s added vehicle types from your application to ensure proper visibility into available rides.
If the vehicle types listed on your Roundtrip application and the roster of licensed vehicle do not match, we manually override and only keep the fleet information that is documented on your company’s roster.
W9 Form
Roundtrip requires your company to provide a W9 Form for billing and tax documentation purposes. A W9 Form is also crucial if your company opts to enroll in direct deposit after your application is approved.
