Click the Book Trip button in the top right corner of your Dashboard
Click the Add a New Patient button
Enter in basic demographic information about the patient. Keep the following in mind:
- The primary phone number should be the number you would like ride notifications sent to
- The primary phone number should be the number that you would like the Lyft driver to contact upon arrival
- You are also able to add a secondary number; This number will be sent the same reminders as the primary and is often used for caregivers or family of the patient. It also can be the number of your facility that you would like to be notified about the ride.
- If the phone number you are entering is unable to receive text messages, make sure you select Automated Voice Call as your notification preference
